If you would like to refer your child for guidance counseling services, please complete the Parent/Guardian Guidance Referral Form below.
The Union Beach School District has a partnership with the YMCA for school-based counseling. If you feel your child could benefit from these services, please complete the referral form below.
If you need to report an incident of bullying to the school, please complete one of the forms below. One form is for families/caregivers and the other is for school employees. All reports of bullying must be documented on the HIB 338 Form.
