Welcome to Memorial School Health office. I am the school nurse at Memorial. Please feel free to contact me at any time regarding the health concerns of your child.  If your child requires medications (inhalers, epi-pens, daily medication etc), please have the medications and the physician's paperwork upon return to school. The forms are located on this site as well as pre-athletic forms.  I look forward to a safe, healthy year with your child.
