SPORTS PHYSICAL INFORMATION

All students enrolled in grades 6-8 may try out for school-sponsored athletic teams. Prior to participation on a school-sponsored interscholastic or intramural athletic team, students enrolled in grades 6 to 12 must have a physical examination and “completed sports package”.
The American Academy of Pediatrics (AAP) advocates that every child and youth receive care through a family-centered medical home (Private M.D.). We therefore strongly encourage you to have your child’s sports physical conducted at your medical home (Private M.D.) as per NJ Administrative Code 6A:16-2.2.
For the “Fall athletic season of the 2020-2021 school year,” the Commissioner of Education will permit any student-athlete who completed a physical examination that allowed the student to participate on a school-sponsored interscholastic athletic team during the 2019-2020 school year to participate in a school-sponsored athletic team, provided that the student-athlete completes the physical examination before the end of the athletic season AND the parent or guardian of the student-athlete provides a proof before the first day of official practice that the physical examination is scheduled. This proof should be a physician’s note stating the date of the physical exam. In addition, the student must provide an updated Health History Questionnaire before the first day of practice.
Please check with the school nurse to determine whether or not your student-athlete has an updated sports physical.
